Report a Product Safety Concern

Your safety matters to MedactiveUSA Corp. If you experience an unexpected reaction or health concern while using one of our cosmetic products, please tell us using the contact form below. This page is for product safety reports, including suspected adverse events.

Seek medical help when needed. Stop using the product if you experience an adverse reaction. For severe symptoms or an emergency, seek immediate medical attention or call 911. This form is not monitored for emergency assistance.

What to include in your report

In the message field, please provide as much of the following information as you can:

• Product name and variant (for example, PHEROLINK Original, Unscented, or Mint + Bergamot)
• Lot or batch number and expiration date, if shown
• Date you used the product and date the reaction began
• Description of symptoms, affected area, and how the product was used
• Whether medical attention was needed and the outcome, if known
• Where and approximately when you purchased the product (optional)

Please include your name and email address in the form so our product safety team can follow up. Do not include Social Security numbers, payment card details, or other unrelated sensitive information.
